Please fill out the registration forms below and submit with the following items to the school office to reserve space for your child. Please do not wait as you may lose your child’s place to another child.

Kinder Preschool report
Most recent report card
Birth certificate
Baptismal certificate
First communion certificate (if applicable)
Immunization records
A doctor health examination report must be submitted for kindergarten and first grade students. (A copy of the form can be obtained from the school office.)
Separated or divorced parents must file a court-certified copy of the custody section of the divorce or separation degree with the principal’s office. The school is not responsible for failing to honor arrangements that have not been make known in writing.
